IN PERSON
Visit the DeKalb County Health Department 8 AM to 12 PM or 1 PM to 4 PM and ask for Vital Records. Complete request form, present current photo identification, and make payment in the form of cash, check, or money order payable to “DeKalb County Health Department.” We also accept major credit/debit cards.

BY MAIL
Submit completed request form and a copy of current photo identification to:
