The program provides caring and confidential staff to help you navigate health care resources.

Medical Case Management Services (pdf) are provided for individuals with Human Immunodeficiency Virus (HIV). In addition, the program provides information and education on Sexually Transmitted Diseases (STDs), as well as treatment for those whose partners are diagnosed with an STD.

There is no fee for medical case management services. Other fees vary depending on services.  Please check with the staff.
