CHF and COPD are
chronic conditions affecting millions. CHF is the underlying reason for
6.5 million hospital admissions each year. Research has shown that
education can reduce the frequency of hospital admissions by teaching
individuals and loved ones how to cope with the day-to-day challenges of
CHF and COPD.
Working with our
physicians, we have designed a program with the goal of reducing
hospitalizations and of maintaining physical conditioning. This is an
education and therapy —based program designed to help:
